Anyone interested in attending the workshop should:

  - Send the completed form to the Workshop Secretariat via fax or e-mail at +82-2-3452-5119, ksr@ksr.or.kr.

  - Early-registration due date: October 15, 2007

• REGISTRATION FEE

Notes

1) Registration for the delegate includes two lunches, welcome reception, coffee breaks, banquet, one book of abstracts and the attendance at all scientific sessions.

2) Student registration will not include banquet.

3) Presenter should complete the registration by August 31, 2007.

• METHODS OF PAYMENT

1) Bank Transfer

Registration fee should be sent to the account below. It is required to fax a copy of transmission with your registration form to the Workshop Secretariat at +82-2-3452-5119. Make sure that registrant's name is provided clearly in the transmission.

2) Credit Card Payment

VISA and MasterCard will be accepted. Please provide us your credit card information including cardholder's name, credit card number and expiration date. Valid signature is required.

* Credit card payment is only available at the ON-SITE REGISTRATION DESK for the KOREAN PARTICIPANTS.

• POLICY OF Cancellation

The cancellation should be made in written notice to the conference secretariat no later than September 30. Full refund except the 20% administration charge will be made. After September 30, no fees will be refunded.

• CONFIRMATION LETTER AND RECEIPT  

Confirmation letter will be sent via e-mail when the registration process is completed (including confirmation of the registration fee). Official receipt will be issued to the registrant at the on-site registration desk during the Workshop (October 21 - 23). If you have any special requests, please contact Workshop Secretariat.

Hotel rooms in Riviera have been booked with specially discounted rate for the workshop participants as below. Workshop secretariat will reserve your rooms when you make a registration for the Workshop. Please indicate your check-in and check-out dates. You may pay your accommodation fees directly to the Hotel when you check out. VAT will not be charged for the foreign visitors followed by the Korean tax law. Breakfast is not included.
